The Gezondheidsverklaring is a CBR questionnaire about your health. The CBR uses it to assess whether you can drive safely, and only once that's settled can we book your practical exam. So sort it out at the start of your training, not just before your exam.
What is the Gezondheidsverklaring?
It's a list of questions about your physical and mental health: your eyes, your arms and legs, conditions such as diabetes or epilepsy, and medicines that can affect your driving.
The CBR checks whether you're fit to drive under the legal requirements. Once the CBR confirms that you're fit to drive, your driving school can book your exam for you.
How to fill it in
You fill it in online at mijn.cbr.nl. If filling it in digitally isn't an option, it can also be done on paper: you collect one at the counter of some exam centres or order it by phone, but that takes a few weeks longer.

Go to Mijn CBR
Go to mijn.cbr.nl and click Inloggen met DigiD (log in with DigiD).

Log in with your DigiD
Log in with your own DigiD, using the DigiD app or your username and password.

Choose the Gezondheidsverklaring
On your start page, click Gezondheidsverklaring rijbewijs invullen (fill in the health declaration).

Choose your licence categories
Tick the categories you want to take an exam for. For the car that is category B.

Answer the questions
You fill in the questions yourself, because they concern confidential information. You're required to answer truthfully.

Check your details
Choose how you want to get post from the CBR, digitally or on paper, and check your phone number and e-mail address.

Onderteken met DigiD
Is everything right? Then tick the box and click Onderteken met DigiD (sign with DigiD). After that you can no longer change the Gezondheidsverklaring.

Betalen
Click Betalen (pay) and pay online. What the Gezondheidsverklaring costs is on the CBR's cost page.

The confirmation
Within an hour you get an e-mail with the confirmation and the answers you filled in. In Mijn CBR you can always see how far the CBR has got with the assessment.
What happens next?

All questions answered with no
If nothing else is needed, you usually hear within an hour that you're fit to drive. After that we can book your exam.
One or more questions answered with yes
That's no problem, but the CBR needs more information. You'll get a referral letter for an examining doctor or a medical specialist, for example, or an invitation for a driving test. The letter says what kind of doctor to see. You pay the doctor's bill yourself, so ask in advance what the examination costs.
If something like that is needed, you'll hear within 4 weeks of submitting. After that the CBR decides within 4 weeks of receiving the information from your doctor or specialist. So count on extra time, and start too early rather than too late.
How long is the decision valid?

You have to take your practical exam within that period. Sometimes a shorter term applies, for medical reasons.

Passed? Then apply for your licence at the gemeente within 1 year. If you don't do that in time, you first have to fill in a new Gezondheidsverklaring.
